Detailed Case Report: Long-term Success in Periodontal Treatment

Introduction:
This case report highlights a significant periodontal treatment journey managed by Ros O’Leary, showcasing the long-term success and effectiveness of periodontal interventions over a 20-year follow-up period.

2001

Patient Profile:

  • Age at Initial Presentation: 38 years old
  • Gender: Female
  • Lifestyle: Non-smoker

Diagnosis:
The patient was referred to us and diagnosed with Generalised Aggressive Periodontitis (GAP) according to the old classification system, or Stage 4 Grade C under current guidelines. This diagnosis highlighted the need for prompt and comprehensive periodontal intervention.

Treatment Plan and Execution:

  1. Initial Approach:
The treatment commenced with two courses of root surface debridement (RSD). The first course consisted of standard procedures, while the second was supplemented with antibiotics—Amoxicillin and Metronidazole—to enhance the therapeutic outcome. The patient’s dedication and motivation were pivotal, resulting in an exemplary response to the initial phase.

  2. Observations and Challenges:
Despite substantial evidence of non-surgical infill throughout many sites, persistent inflammation and deep periodontal pockets (exceeding 8mm) remained at the LL6 region. These pockets posed a considerable challenge, necessitating further intervention beyond conventional methods.

  3. Advanced Surgical Intervention:
Given the persistent issues at LL6, a localised regenerative surgery was deemed necessary. The procedure involved the application of Bio-Oss (a bone substitute) and Bio-Guide (a collagen membrane) to promote regeneration and stability in the affected area. The surgery resulted in an excellent response and demonstrated expected partial infill, significantly improving the periodontal status.

2021 – 20-Year Follow-up

Outcome and Long-term Follow-up:
Over the subsequent 20 years, the patient maintained remarkable periodontal health, validating the initial treatment strategies and interventions. The long-term follow-up illustrated the enduring success of the regenerative procedure, emphasising the importance of patient motivation and comprehensive management of contributing factors.

Learning Outcome:
This case illustrates that localised regenerative surgery can effectively address complex periodontal issues in highly motivated patients. When all contributory factors are meticulously managed, it reinforces the potential for achieving sustained periodontal health and stability.
