DENTAL EDUCATION: Long-Term Implant Success in a Bruxer with Periodontal History
How do you manage full molar loss in a patient with bruxism and a history of periodontal disease? In this 10+ year follow-up, Dr. Chong Lim shares a complex case that combines sinus grafting, occlusal protection, and strict maintenance to achieve lasting implant success. See how thoughtful planning and patient compliance led to remarkable long-term stability — even under challenging conditions.
10+ Year Follow-Up Case Study
Clinician: Dr. Chong Lim
Case Duration: 2006 – 2024
Patient Profile: Male, Bruxer with Periodontal History
Restorative Region: Posterior Maxilla (Molars)
Clinical Challenge
This patient presented in 2006 having lost all upper molars & 3 premolars, due to advanced periodontal disease, combined with a history of bruxism. The posterior maxilla also exhibited pneumatisation of the maxillary sinuses, leaving insufficient bone volume for conventional implant placement.

Treatment Plan & Solution
A staged approach was taken to restore function and long-term health:
- Periodontal Control:
Initial treatment focused on stabilising periodontal health in the remaining teeth through debridement, maintenance, and tailored hygiene instructions. - Occlusal Management:
The patient was provided with a full-coverage occlusal splint to protect future prosthetics and manage parafunctional forces due to bruxism. - Sinus Grafting:
Bilateral lateral window sinus grafting was performed to regenerate adequate bone volume for implant placement. Bone measurements confirmed vertical gain sufficient to place implants of appropriate length and diameter. A combination of the patient’s own bone & xenografts were used.

Implant Placement:
Implants were placed in simultaneously and following 8 months when osseointegration was achieved, final splinted crowns were placed.

Mitigating Factors for Success
- Excellent Oral Hygiene: The patient demonstrated strong compliance with hygiene instructions.
- Routine Maintenance: Scheduled follow-up visits every 6 months enabled ongoing peri-implant health assessment and early intervention where needed.
- Protective Protocols: Continued use of the occlusal splint protected the restoration over time from excessive loading.
Clinical Outcome
Radiographic evaluation at 10+ years post-placement (2024) shows;
- Stable crestal bone levels
- Intact sinus graft volume
- No signs of peri-implantitis
- Prosthetics remain functional with no major complications

This case demonstrates the importance of a multi-disciplinary approach, patient compliance, and rigorous long-term maintenance in managing high-risk patients with a history of bruxism and periodontal disease.
